Anaesthesiology, Reanimation and Intensive Care Department

The presence of this department in the structure of the center allows to significantly extend the volume of complex instrumental investigations with less risk for patients, including general anesthesia at endoscopic investigations (esophagogastroduodenoscopy, colonoscopy) without any discomfort in anesthetic sleep.

The most modern and safe medicinal facilities are used in everyday work. Anesthesiology, Reanimation and Intensive Care Department provides help at difficult operative interventions for patients with the expressed concomitant pathology, including for patients with a sepsis.

A highly skilled personnel works in a department, including 2 candidates of medical sciences, 5 highest category physician. Categorization composes – 100%. The department is equipped with the newest equipment for performance of anesthetic and reanimation care. Specialists of the department work on new methodologies of modern regional and general anesthesia.

Being highly qualified specialists, the Department staff is ready to immediately give help in the most difficult clinical situations.

High professional training of the personnel and technical equipment of the department allows to conduct intensive monitoring and treatment of patients after the most complex and unique surgeries, and perform timely qualified resuscitation. During the 15-year period, the department developed and introduced into clinical practice the basic principles of patients management after reparative operations on the esophagus, gastrointestinal tract, liver, biliary tract, pancreatic gland.

Currently, along with the development of such new surgical technologies as related organ transplantation (liver, kidneys), extensive liver resection, pancreatoduodenal resection, the principles of these patients management in the perioperative period are constantly improving.

The basic tasks of the anaesthesiology and reanimation department are the following:

• twenty-four-hour provision of anaesthetic manuals at planned surgeries of different level of complexity (including during transplantation of liver and kidney from a live relative donor);
• twenty-four-hour provision of anaesthetic manuals during emergency surgeries of different level of complexity (including transplantation of liver and kidney from cadaver);
• emergency medical treatment, intensive therapy, resuscitation aids;
• preparation of heavy contingent of transplantation patients to operative interventions, painful manipulations and invasive diagnostic investigations.
• intensive therapy and reanimation of different level patients undergoing treatment at JSC «NSMC».

The reanimation department, unfolded on 12 beds (including 6 beds of post anesthesia care unit, where dynamic supervision over patients after short-time operative interventions not exceeding 2-3 hours is performed), conducts treatment of patients after vast interventions on the organs of pectoral and abdominal cavity, and small pelvis. Also, there are 2 isolators for septic patients in the department.

We are currently working on the following practical directions:

• Technology of patient protection from pain at all stages of the disease development and treatment;
• technology of the anesthetic provision and perioperative intensive therapy at general surgery;
• technology of safe anesthetic provision of geriatrics contingent of patients with heavy concomitant cardiovascular pathology in surgery;
• technology of safe anesthetic provision during liver and kidney transplantation;
• diagnostic and curative problems in surgical patients with a high risk of thrombotic and bleeding complications at the surgical treatment stages;
• features of perioperative management of patients with a high risk of cardiac rhythm and conduction disorder development;
• development of prophylaxis methods of postoperative cardiac rhythm disorder in surgery;
• development of modern prophylaxis charts for postoperative nausea and vomiting in surgery;
• perioperative management of patients with nutritional insufficiency.

In addition, the department conducts training of specialist-doctors within the framework of residency, advanced training, retraining in the specialty of anesthesiology, reanimation and intensive therapy at the workplace.
